Services
Unity Care services benefit the Santa
Clara, San Mateo, and Monterey counties and their extended San
Francisco Bay Area communities. We focus on each child as an
individual, and offer a full range of services that empower young
people and provide the security and support they need to flourish in
society.
Community Based Services:
- Hip Hop 360 – Hip-Hop
360 provides middle school and high school students the opportunity to
express themselves through various artistic avenues.

Independent Living Program (ILP)
– Through group workshops, individual coaching and goal-setting, ILP prepares our residents for that transition to self-sufficiency.

Reaching Independence
through Sustainable Employment (RISE) - The RISE Program serves youth who are
at-risk and struggling to succeed in an educational and/or work environment
due to various factors.
- Transition to Independence Process (TIP) - Serves youth and young adults ages14-24, who are experiencing emotional or behavioral difficulties by assisting youth to make a successful transition into adulthood

Y-Tribe
Program
– A year-long program that provides training and education to address
the social developmental challenges our youth face.
Mental Health Services:
- Community Clinic –
Provides a full array of quality mental health services to San Jose area youth and families currently receiving Unity Care services.
- The Nia Project – Aims to help and strengthen African American families.
- Therapeutic Behavioral (TBS)
– One-on-one mental health services for children with severe behavioral
issues.
- Project
Odyssey (Wraparound Services)
– Maintain the family bond by reuniting the children with their
family and home community. Wraparound establishes family teams to create
and implement plans that meet the specific needs of each family.
Residential Services:
- Residential
Treatment Homes – Nurturing home environments that represent a healthy neighborhood
household. Our homes are located in vibrant middle-class communities to
ensure a welcome and safe environment.
Former Foster Youth Services

Transitional Housing Placement Plus
(THP+) – A safe and healthy home environment for young
adults, ages 18-24, who are required to leave foster care because they
reach legal adulthood and as a result become homeless.
Family Support Services
- Parent Advocate Services - Assists over 200 parents in Santa Clara County who are currently in the family reunification process for up to 15-18 months.
